Community
Tobacco Cessation Initiative Grant
Toledo-Lucas County Health
Department (TLCHD) has been awarded the Community Cessation
Initiative (CCI) grant from the Ohio Department of Health
aiming to decrease tobacco use county-wide and increase
access and knowledge of cessation services to Lucas County
residents. The objective of this grant is to create a
referral network to tobacco cessation services in Lucas
County in order to best help potential patients reach the
appropriate services easily, with limited barriers. Through
this referral network, TLCHD will be able to complete
appropriate follow-up and relapse management after treatment
is complete, ensuring the best possible quit attempt.
As part of this
initiative, TLCHD is working to identify and bridge the gaps
in current cessation services offered in Lucas County.
Additionally, we are focusing on three disparate
populations: African Americans and Latinos in low
socioeconomic communities, as well as pregnant women.
Toledo-Lucas County Health
Department is looking for as many community partners as
possible who would be interested in contributing their
services and/or referrals to the CCI network. These
community partnerships can include provider partnerships,
referral partnerships, or both.
